Region 20 Transition Program

The Region 20 Transition Program is designed to support students with disabilities as they prepare for life after high school. The program provides individualized opportunities for students to build the skills, confidence, and experiences they need to pursue their personal goals and become as independent as possible.

Key Features of the Program:

  • Individualized Support: Develop personalized transition goals and supports based on each student’s strengths, interests, needs, and future plans.

  • Career and Vocational Development: Provide opportunities to explore careers, develop job-readiness skills, participate in work-based learning, and gain meaningful employment experience.

  • Independent Living Skills: Teach practical skills related to daily living, self-advocacy, decision-making, communication, and personal responsibility.

  • Community Integration: Encourage students to participate in community-based activities and experiences that promote social connections, confidence, and independence.

  • Post-Secondary Planning: Help students prepare for their next steps, whether that includes employment, post-secondary education or training, supported living, or other individualized goals.

The goal of our Transition Program is to empower each student to build a meaningful future by developing the skills, experiences, and self-advocacy abilities necessary to achieve their goals and participate as independently as possible in their community.
